Employee Fee Waiver Application

The Employee Fee Waiver Application is a formal request to participate in the educational benefit afforded to employees based on employment status. This form must be submitted each term of participation specifying course work which supports goals outlined on the Individual Career Development Plan (ICDP). New fee waiver participants must apply for admission to Cal Poly.

An employee may be given release time to attend one course per quarter, if the supervisor/manager determines that the operations of the department will not be adversely affected by the employee's absence. "Release time" means that an employee has supervisory or managerial approval to attend a class during the employee's normal work hours. Normal work hours may be adjusted with supervisory or managerial approval.

The Employee Fee Waiver Course Modification form must be submitted if the course schedule changes or if withdrawing from all classes.

Applicant Instructions

  1. After launching the Employee Fee Waiver Application, a DocuSign PowerForm will prompt you for your name and email. Next, you will enter the names and emails of the individuals who will be reviewing the application. Note: If you know any of the reviewers will be out of the office when the form is being routed, you may replace the reviewer with another individual who has designated authority to sign during their absence.
    • Supervisor: This is the person you report to on a daily basis, they may be a lead or manager.
      • Will determine if release time to attend is appropriate based on operational needs (one course per term).
      • Leave this role blank if "Supervisor" and "Director/Dept. Head" are the same individual.
    • Director/Dept Head: Management individual for your specific area.
      • The person that will determine if the course(s) are "Work Related" or for "Career Development".
      • If same as "Supervisor" they will determine if release time to attend is appropriate based on operational needs (one course per term).
    • Division Head/Dean: Tyipically a Vice-President, Dean, or Associate Dean.
  2. Press the "Begin Signing" button and complete all required form fields.

Reviewer Instructions

Supervisor

  • If applicable, reviews operational needs to determine if release time for one course per term is feasible.

Director/Dept Head

  • Review courses to determine if "Job Related" or "Work Related" based on employee's Job Description and Individual Career Development Plan.
    • Work Related – courses which have a direct relation to the employee’s current position; taken to enhance skills required to perform current job duties or newly assigned tasks.
    • Career Development – courses which enhance skill for the purpose of career growth within the CSU.
  • If applicable, reviews operational needs to determine if release time for one course per term is feasible.

Division Head/Dean

  • Review and approve if applicable.
