Open Enrollment - Your opportunity to review, enroll, cancel or make changes in the following plans:

  • Health Care Reimbursement Account (HCRA) – Must re-enroll each year
  • Dependent Care Reimbursement Account (DCRA) – Must re-enroll each year
  • Enroll or Disenroll in voluntary plans – MetLaw, Vision Services Premier Plan
  • Enroll/change/cancel your health and/or dental plan
  • Add/remove eligible dependents from your plans
  • Enroll in Flex Cash in place of health/dental coverage

2020 Open Enrollment Guide (published by the CSU - some Health plans listed not available in our area)

Notable Changes:

  • All HMO premiums will increase. UnitedHealthcare's increase will be very small.
  • Annual maximum contribution for HCRA will increase to $2,700 ($225.00/month).
  • AFLAC Critical Illness to be discontinued 01/01/2020. The Standard will offer a voluntary critical illness plan beginning February 2020. Current enrollees may stay with AFLAC via direct pay or transfer to the new coverage with 'The Standard' in 2020.
  • New! Pet Insurance available effective 01/01/2020.
  • New! Accident Insurance through 'The Standard'- Special enrollment period early 2020.
  • New! Added value feature from Delta Dental: Hearing aid and Lasik service discounts.Click here to view flyer. Hearing discount through VSP still also available.

If you make changes:

  • You will receive new health plan ID cards if you change your health plan; make sure your mailing address is correct in your portal.
  • If you change plans or enroll in HCRA/DCRA and you don't see the correct deductions applied to your February pay check, contact us at 756-2236.

Employee contributions are made on a pre-tax basis as permitted through the CSU Tax Advantage Premium Plan (TAPP). If you would like your employee contributions taxed, please contact Human Resources.

    CalPERS

    CalPERS Online Health Plan Statement

    All health plan members can compare plans, view, download, and/or print their CalPERS 2020 Health Plan Statement at https://my.calpers.ca.gov (log in as participant). This feature will provide employees with customized health information regarding health benefit changes that may impact them.

    Dental Plans

    Cal Poly continues to offer an HMO and PPO through Delta Dental. Premiums for dental plans continue to be fully paid by Cal Poly for employees and covered dependents.

    Health Care Reimbursement Account (HCRA) & Dependent Care Reimbursement Account (DCRA)

    Important reminder: HCRA and DCRA deductions do not automatically continue into the next plan year. If you want deductions to continue for 2020, you must submit your elections via My Cal Poly Portal during the Open Enrollment period.

    HCRA is a voluntary benefit plan offering eligible employees the ability to pay for qualifying out-of-pocket health care expenses with pre-tax dollars. Employees may elect a monthly deduction amount of as little as $20 per month up to a maximum $225.00 per month. For more information, please review the HCRA brochure.

    DCRA is a voluntary benefit plan offering eligible employees the ability to pay for qualifying out-of-pocket dependent care expense with pre-tax dollars. Employees may elect a monthly deduction amount of as little as $20 per month up to a maximum of $416.66 per month. For more information, please review the DCRA brochure.

    Empathia Life Matters Employee Assistance Program

    Employee Assistance Programs is an assessment, short-term counseling and referral service designed to assist you and your family in managing everday concerns. In addition to in-person EAP counseling, LifeMatters offers phone counseling sessions by appointment. These sessions may be scheduled through the program's toll free number and are conducted by providers located and licensed in the State of California.     Nationwide Pet Insurance

    Nationwide Pet Insurance - Enroll during Open Enrollment or anytime after 01/01/2020

    New pet health insurance gives you the freedom to use any vet, anywhere, including specialist and emergency providers. Nationwide is offering two benefit option for your pets. This insurance can cover your pet's accidents, illness and even preventative care and wellness services. Plans are available for dogs, cats, birds, small mammals and exotics (such as reptiles).

    Hyatt Legal Plans

    MetLaw Legal - Enroll or Cancel only during Open Enrollment

    For $21.70 per month, the MetLaw Legal plan offered by Hyatt Legal Plans, Inc., provides representation for many personal legal services for employees and their eligible dependents.

    Open enrollment is your annual opportunity to enroll or discontinue enrollment in the MetLaw Legal plan. Current employees that do not enroll in the plan during open enrollment will not have another opportunity to enroll in the plan until next year's open enrollment period.

    VSP

    Vision Premier Plan - Enroll or Cancel only during Open Enrollment

    Employees may purchase Vision Service Plan (VSP) Premier coverage as an enhancement to the basic coverage. The VSP Premier plan offers a higher level of benefits for lenses, contact lenses and frames.

    Employees may enroll in or cancel their VSP Premium Plan by calling (800) 400-4569 or online at http://csuactives.vspforme.com only during Open Enrollment. (Employees will continue to be automatically enrolled in the employer paid VSP Basic Plan.)

    Premiums for the VSP Basic Plan will continue to be fully paid by Cal Poly for employees and their dependents. If an employee enrolls in the VSP Premier plan, all benefits eligible dependents must also be enrolled or they will be removed from the basic plan.

    The Standard

    Voluntary Life Insurance or Long Term Disability Insurance - Apply anytime

    The Standard offers Group Life Insurance as well as Accidental Death and Dismemberment (AD&D) coverage for you and your eligible dependents.

    Voluntary Long Term Disability (LTD) administered by The Standard provides loss of income protection (up to 60%) due to a qualified disability. They offer two different plan options which include a 30 or 90 day waiting period. Eligible employees (those not covered by The Standard LTD employer paid plan) can apply at any time during employment.

    Employees who chooses to enroll in Group Life or LTD after the first 60 days of hire or eligibility, must submit a completed medical questionnaire along with the enrollment form.

    California Casualty

    California Casualty - Enroll anytime

    The Auto, Home and Renter's Insurance plan offered by California Casualty allows CSU eligible employees to obtain auto and home insurance policies at a discounted rate. The plan also has an identity theft component known as ID Defense. All policies are written for a 12-month period. Eligible employees can enroll in this benefit at any time during employment.

    Scholar Share

    Scholar Share - Enroll anytime

    A 529 plan is an account that allows you to invest specifically for future education expenses. 529 plans are aimed at helping families save for college. The accounts are administered at the state level, and are managed by a financial services company, TIAA-CREG Tuition Financing Inc. You can open an account with as little as $25 dollars per investment portfolio, or $15 dollars per pay period through payroll deduction.

    CalPERS Long Term Care - Apply anytime

    CalPERS Long-term care (LTC) coverage helps pay for the high cost of care for assistance with the activities of daily living due to illness, injury, disabilty, cognitive disorder or aging. CalPERS LTC is an optional, employee-paid benefit available to all current and former members (including retirees) and their eligible family members, including:

  • Parents-in-law,
