Open Enrollment for the 2019 Plan Year is Now Closed
- All current health, dental and vision plans will continue to be offered in 2019.
- UnitedHealthCare HMO premiums have decreased and all other health plan premiums have increased.
- Office visit co-pays have increased for PPO plans from $20 to $35 for urgent care and specialists.
- PERS Select PPO has adopted a value-based design with signigicant change to deductibles, co-pays, and share of cost for certain services.
- Delta Dental PPO preventative services no longer count against annual maximum benefit, and SmileWay Wellness Benefit offers expanded coverage for those with certain health conditions.
- Vision Service Plan continues to offer voluntary Premier Plan upgrade.
- Annual maximum contribution for HCRA has increased to $2,650 ($220.83/month).
2019 Plan Rate Changes
Cal Poly pays the majority of the health plan premium costs for benefits eligible employees. Employee contributions are made on a pre-tax basis as permitted through the CSU Tax Advantage Premium Plan (TAPP). If you would like your employee contributions taxed, please contact Human Resources.

Bargaining Unit 6, State Employee Trade Council (SETC), CSU pays slightly LESS: Deduct an additional $5.00 /$10.00/$20.00 from the employee contribution dependent on number of enrollees in plan.

CalPERS Online Health Plan Statement
All health plan members can view, download, and/or print their CalPERS 2019 Health Plan Statement at https://my.calpers.ca.gov (log in as participant). This feature will provide employees with customized health information regarding health benefit changes that may impact them.
Cal Poly continues to offer an HMO and PPO through Delta Dental. There has been one change to the PPO plan. For PPO plans, all preventative care cost will not count towards the maximum annual benefit. Premiums for dental plans continue to be fully paid by Cal Poly for employees and covered dependents.

Health Care Reimbursement Account (HCRA) & Dependent Care Reimbursement Account (DCRA)
Important reminder: HCRA and DCRA deductions do not automatically continue into the next plan year. If you want deductions to continue for 2019, you must submit your elections via My Cal Poly Portal during the Open Enrollment period.
HCRA is a voluntary benefit plan offering eligible employees the ability to pay for qualifying out-of-pocket health care expenses with pre-tax dollars. Employees may elect a monthly deduction amount of as little as $20 per month up to a maximum $220.83 per month. For more information, please review the HCRA brochure.
DCRA is a voluntary benefit plan offering eligible employees the ability to pay for qualifying out-of-pocket dependent care expense with pre-tax dollars.
