2025 Open Enrollment Highlights
The below items are notable changes and enhancements for the 2025 plan year
- PPO Medical Plan Vendor Change: PERS Gold and PERS Platinum plan administration
is moving from Anthem Blue Cross to Blue Shield and Included Health. Benefits under the plan are
not impacted. Contact Included Health at (855) 633-4436 to verify provider network participation.

- Benefit Enhancement to all Medical Plans: Doula benefit for all pregnant
members; Postpartum benefit for Basic members; Travel benefit for medically necessary care.
- Benefit Enhancement for PORAC: Combined 20-visit limit for acupuncture and
chiropractic visits; No visit limit on physical and occupational therapy.
- HCRA Maximum Contribution Increase: Annual Max $3,200; Monthly Max $266.66
- Special Voluntary Life One-Time Special Enrollment Opportunity with The Standard:
Guarantee Issue without medical certification up to $100,000 employee coverage and
$50,000 Spouse/Domestic Partner. If previously declined after submitting Evidence of
Insurability, new EOI must be submitted for consideration.

Cal Poly continues to offer an HMO and PPO plan choice through Delta Dental. Premiums for dental
plans continue to be fully paid by Cal Poly for employees and covered dependents.

Vision Plans
Premiums for the VSP Basic Plan will continue to be fully paid by Cal Poly for employees and their
dependents. If an employee enrolls in the VSP Premier plan, all benefits eligible
dependents must also be enrolled or they will be removed from the basic plan.
Employees may purchase Vision Service Plan (VSP) Premier coverage as an enhancement to the basic

Flexible Spending Accounts: Health Care Reimbursement Account (HCRA) & Dependent Care Reimbursement
Account (DCRA)
Important reminder: HCRA and DCRA deductions do not automatically continue into the next
plan year. If you want deductions to continue for 2025, you must submit your elections
via My Cal Poly Portal before Oct 11.
HCRA is a voluntary benefit plan offering eligible employees the ability to pay for qualifying
out-of-pocket health care expenses with pre-tax dollars. Employees may elect a monthly deduction
amount of as little as $20 per month up to a maximum $266.66 per month ($3,200 annual). For more information, please

DCRA is a voluntary benefit plan offering eligible employees the ability to pay for qualifying
out-of-pocket dependent care expense with pre-tax dollars. Employees may elect a monthly deduction
