CSU Paid Parental Leave
Maternity, Paternity, and Adoption leave, more commonly referred to as “Parental Leave,” is leave associated with the birth of an employee’s own child or the placement of a child with the employee in connection with adoption or foster care. Eligible employees may qualify for up to 30 workdays of paid parental leave starting within 60 days of the arrival of a new child. This may vary for select employee groups. If possible, to coordinate Parental Leave benefits, all employees must complete the Request for Leave form at least 30 days in advance .
Request for Leave (required for all leaves) ADOBE SIGNPregnancy Disability Leave
Pregnancy Disability Leave (PDL) is provided to eligible pregnant employees. If an employee is disabled by pregnancy, childbirth or a related medical condition they are entitled to up to four months of disability leave per pregnancy and leave can be taken consecutively or intermittently. PDL may be modified as an employee’s changing medical conditions dictates. The employee’s health care provider determines how much PDL will be needed and is counted against the four-month period.

All employees, full and or part-time, requesting Pregnancy Disability Leave and/or accomodations due to pregnancy, must complete the medical certification below and provide it to your healthcare provider to complete and return to Cal Poly Human Resources, Disability Leaves fax 805-756-5483, or disabilityleaves@calpoly.edu
